Progress has been particularly rapid in the past few years when M-mode echocardiography, and more recently 2-dimen sional echocardiography have been applied to the study ofHCM. In addition to new insights as to what the disease is, there has been enormous progress concerning its treatment, with the application of beta-adrenergic blocking agents and surgical relief of left ventricular outflow tract obstruction. Although these approaches have led to great strides in the symptomatic control of the disease, many patients' symptoms have remained refactory to medical and surgical therapy. Most discouragingly, sudden death still occurs, even in patients on large doses of beta-blocking agents and in patients who have had surgical relief of left ventricular outflow tract obstruction.

Members of this working-group have tried to study the many aspects of this intriguing disease, and it is in this volume that they present their recent results and current concepts (chapters 2, 3, 5 and 6). A contribution from the Dutch interuniversity Institute of Cardiology presenting a detailed account of evaluation by means of radionuclide techniques completes the non-invasive picture of cardiomyopathies (chapter 4). On July 6th, 1984, a symposium entitled 'Recent views on hypertrophic cardio myopathy' was held at the University Hospital Groningen. The main reason to organize this meeting was the retirement of one of our working-group members, namely E. van der Wall, as head of the cardiological department. The speakers at the symposium presented their lectures based on their contributions to this book. It is an honour for us to have obtained their full cooperation for this book and for the symposium. We are very grateful that this Groningen manifestation could include the outstanding contributions of several well-known experts from abroad. We there fore thank our colleagues Profs. In 50-60% of adolescents and adults with HCM, the disease is inherited as an autosomal dominant trait caused by mutations in cardiac sarcomere protein genes. The most cases are due to mutations in genes which determine the synthesis of myosin-binding protein C (MYBPC3) and beta-myosin heavy chain (MYH7). More rarely involved genes are those encoding myosin light chain 3 (MYL3), tropomyosin alpha-1 chain (TPM1), and cardiac troponins I and T (TNNI3, TNNT2). Mutations in genes encoding Z-disc or calcium-handling proteins account for less than 1% of cases. Multiple sarcomeric protein mutations are present in up to 5% of individuals. A further of 5% of patients have inherited metabolic or neuromuscular diseases, chromosome abnormalities, and genetic syndromes. HCM is characterized by a highly heterogeneous phenotype, highly variable intra- and interfamily expressivity and incomplete penetrance, therefore by a genotype-phenotype plasticity.
